Do I need to activate my savings card before going to the pharmacy? If yes, what is the activation number for this offer?

Yes, you will need to activate your card before going to the pharmacy. To activate please call: 888-537-3634 and select prompt #2 for assistance.

Can this savings card offer be used for prescriptions reimbursed under Medicaid, Medicare, TRICARE, or any prescription drug benefit plan?No. If you are eligible for drug benefits under any such federal or state drug assistance programs, you cannot use this offer. I have Medicare/Medicaid/Tricare, but my prescriptions are not covered, can I use the savings card offer?No, if you are eligible for drug benefits under any federal or state drug assistance programs, you cannot use this offer. This includes patients who may fall into the ‘doughnut hole’ or those who are paying for 100% of the prescription out-of-pocket.

If I am enrolled in the Affordable Care Act (or 'Obamacare'), am I eligible to use the savings card offer?This offer is available to patients covered by Qualified Health Plans issued by Federal or State Exchanges created under the Affordable Care Act. Independent verification of offer availability by the healthcare provider is required for non-Qualified Health Plans issued by the Exchanges.

I have commercial insurance, but KERENDIA is not covered by my plan. 
Am I eligible for the KERENDIA Savings Card?
KERENDIA Savings Card applies to all eligible commercial patients regardless of your coverage for KERENDIA.
I am under 18 years of age. Is my policy holder for my insurance eligible to participate in this program?Yes, the insurance policy holder is eligible to participate in this program.
I received the KERENDIA Savings Card does this mean I do not need to have a prescription?No. The KERENDIA Savings card is for use ONLY with a valid prescription.
Is there a maximum amount the savings card offer will cover per use?The offer will cover up to $3,000 per year for eligible patients.
If my mail order pharmacy doesn’t accept the savings card offer, can I mail in for a reimbursement?

Yes. You can get your savings via a rebate check after you have purchased your KERENDIA prescription via mail order. In order to receive your check, please provide the following information:

 

The 11-digit ID# and GRP# that is found on the KERENDIA Savings Card or a photocopy of the card. A proof of purchase (original pharmacy receipt with a pharmacy name, product name, prescription number or Rx#, date filled, quantity and the price). If applicable, provide a legible photocopy of the front of your primary insurance card or provide the name of your primary insurance along with BIN and PCN information found on the card. Your name, address, city, state, zip, phone number, DOB and the out of pocket payment. Mail all of this information to: KERENDIA Savings Program Claims Processing Dept., PO Box 2355 Morristown, NJ 07962.

Can the free trial voucher be used for prescriptions reimbursed under Medicaid, Medicare, TRICARE, or any prescription drug benefit plan?

The free trial voucher is available to all new patients with a valid prescription regardless of the type of insurance you have.

Patients are eligible for the co-pay card if they are commercially insured and may pay as little as $0 and save up to $3,000 per year. Patients who are enrolled in any type of government insurance or reimbursement programs are not eligible. As a condition precedent of the co-payment support provided under this program, e.g., co-pay refunds, participating patients and pharmacies are obligated to inform insurance companies and third-party payers of any benefits they receive and the value of this program, and may not participate if this program is prohibited by or conflicts with their private insurance policy, as required by contract or otherwise. Void where prohibited by law, taxed, or restricted. Patients enrolled in the Bayer US Patient Assistance Foundation are not eligible. Bayer may determine eligibility, monitor participation, equitably distribute product and modify or discontinue any aspect of the KERENDIA $0 Co-Pay Program at any time, including but not limited to this commercial co-pay assistance program.

KERENDIA free trial voucher may provide new patients with up to a 90-day supply of KERENDIA and must be accompanied by a valid, signed prescription for KERENDIA. This offer is valid only in the United States and Puerto Rico. Voucher Program shall not apply in any state or jurisdiction where prohibited. KERENDIA supplied free of charge through the Voucher Program is not contingent on continued use of KERENDIA or any other prescriptions or use of Bayer products. No claim for reimbursement for product dispensed pursuant to this voucher may be submitted to any patient or third-party payer whether a commercial or a government payer. Not valid if reproduced. It is unlawful for any person to sell, purchase, trade, barter or export KERENDIA supplied through the voucher program or make an offer to do so. This free-trial voucher cannot be combined with any other rebate/offer, free trial or similar offer for the specified prescription. Limit 1 voucher per patient. You must be 18 years old or older. Bayer reserves the right to change or discontinue this Voucher Program at any time without notice.
